DESCRIPTION:Are you living with a chronic health condition or caring for 
 someone who is? This six-week series of small group sessions will HeLP you 
 put life back into your life!  Geared toward ages 55 and older and/or their 
 caregivers\, this series of six weekly 2.5-hour in-person sessions offers a 
 sensible and enjoyable way to enhance your quality of life by showing you 
 how to manage conditions such as high blood pressure\, pain\, depression\, 
 arthritis\, and diabetes.\n\nThe Health for Life Program (HeLP) workshop 
 series follows a model developed at Stanford University’s School of 
 Medicine\, an internationally recognized and proven program that benefits 
 participants. Workshops are facilitated by specially trained and certified 
 volunteers\, not necessarily health care professionals\, but frequently 
 people who are managing chronic conditions themselves. HeLP does not 
 replace your healthcare team. Rather\, it enables you to work more 
 effectively with health providers to enhance your health outcomes.\n\nThe 
 workshop series is guided by the book\, Living a Healthy Life with Chronic 
 Conditions (5th edition)\, a copy of which will be provided for program 
 participants. The book is also available to borrow through the Westchester

 Spanish\, Tomando Control de su Salud\, in both print and ebook formats.
