Managing Chronic Conditions - Health for Life Program (HeLP) series In-Person

Are you living with a chronic health condition or caring for someone who is? This six-week series of small group sessions will HeLP you put life back into your life!  Geared toward ages 55 and older and/or their caregivers, this series of six weekly 2.5-hour in-person sessions offers a sensible and enjoyable way to enhance your quality of life by showing you how to manage conditions such as high blood pressure, pain, depression, arthritis, and diabetes.

The Health for Life Program (HeLP) workshop series follows a model developed at Stanford University’s School of Medicine, an internationally recognized and proven program that benefits participants. Workshops are facilitated by specially trained and certified volunteers, not necessarily health care professionals, but frequently people who are managing chronic conditions themselves. HeLP does not replace your healthcare team. Rather, it enables you to work more effectively with health providers to enhance your health outcomes.

The workshop series is guided by the book, Living a Healthy Life with Chronic Conditions (5th edition), a copy of which will be provided for program participants. The book is also available to borrow through the Westchester Library System in printe-book, and e-audiobook formats as well as in Spanish, Tomando Control de su Salud, in both print and ebook formats. 

Registration enrolls you in the full six-week series held on Mondays, October 28, November 4, 18, and 25, and December 2 and 9 (NO CLASS on November 11) from 1-3:30 PM. IMPORTANT NOTE: You must make every effort to attend ALL SIX weekly workshops, as the material covered builds upon the prior sessions. Additionally, the PARAMETERS OF THE LICENSE FOR THIS WORKSHOP REQUIRE PARTICIPANTS TO ATTEND AT LEAST FOUR SESSIONS, INCLUDING THE FIRST AND/OR SECOND SESSION. NO NEW PARTICIPANTS WILL BE ADMITTED AFTER THE SECOND SESSION.

REGISTRATION FOR THIS WORKSHOP SERIES IS FULL WITH A SIGNIFICANT WAITLIST.

This workshop series is part of the Health for Life Program offered by the  Westchester County Department of Senior Programs and Services in partnership with Westchester Public Private Partnership for Aging Services, Westchester County Livable Communities, and the New York State Office for the Aging.
